An up-to-date physical form needs to be on file with the school nurse in order to be able to participate in sports.

• All physicals are valid for 13 months after the well visit.

• When submitting a physical electronically, be sure the entire document is submitted and includes your child's name and the date of the well visit.

• Any physical submitted that is missing these items will prohibit your child from being cleared for participation.

• Physical exams from CVS Minute Clinic or Convenient MD are acceptable. 

There are several ways that you can submit your physical:

1. For the Fall Season: Physicals should be uploaded during the sport registration in Step Two.

   • The Fall season begins prior to the Nurses first day of school.

2. For the Winter and Spring: Physicals should be brought or emailed directly to the Andover School Nurses Office.

   • Our school nurses are: Heidi Katz and Melissa Sucharewicz

3. Physicals that expire during the season require a valid physical to be submitted directly to the School Nurse's Office in order for your child to continue participation.
